CHAMPVA reimbursement claim
If a provider does not bill CHAMPVA, pay first and file for reimbursement. Direct deposit through the Financial Services Center is required for payment to you.
Who it is for. Already enrolled in CHAMPVA. TRICARE-eligible people cannot use CHAMPVA.
Bring. VA Form 10-7959A, paid receipt, itemized bill (CPT/HCPCS and diagnosis codes). Other insurance: explanation of benefits. Pharmacy claims need NDC, dosage, and fill date.
Timing. Usually 1 year from the date of care, or 1 year from hospital discharge.
